Strep Throat: What Parents and Caregivers Need to Know

All sore throats are not created equal. When a sore throat is caused by a virus, for听example, it tends to resolve on its own without complications, and at-home remedies will听usually provide relief along the way. Strep throat鈥攁lso known as strep pharyngitis鈥攊s a听听very different matter.听
Caused by the bacteria Streptococcus pyogenes, (also called Group A Streptococcus),听strep throat can be serious, especially if left untreated. Some of its complications include tonsillar abscesses, acute rheumatic fever, rheumatic heart disease and kidney inflammation, among others. That鈥檚 why it鈥檚 so important to diagnose and treat a child鈥檚 strep throat promptly and effectively.听
Strep throat usually affects children 3 years and older, says Dr. Brittany Sakhno, a听Brooklyn-based pediatrician and instructor in pediatrics at 日本av色情片.听
The telltale symptoms of strep throat include the following:听
- Sore throat听
- Abdominal pain听
- Fever听
- Headache听
- Swollen lymph nodes in the neck听
鈥淏y contrast, a cough, congestion and runny nose are usually not present when a child听has strep throat, and are more typical of a viral infection,鈥 says Dr. Sakhno. As a parent,听bring your child in for testing if you are concerned.听
How do you diagnose strep throat?听
鈥淢ainly, we look for significant redness in the back of the throat, little red dots on the听roof of the mouth (called petechiae) and white spots (called exudates) on the tonsils,鈥听Dr. Sakhno says. 鈥淪ometimes, we see a rough rash on the child鈥檚 skin that feels like sandpaper and a bumpy red tongue that resembles the skin of a strawberry; these are signs of scarlet fever.听
鈥淪trep throat is highly contagious,鈥 she adds. 鈥淭herefore, we ask parents and caregivers whether anyone else in the family or any of the child鈥檚 classmates has a strep infection.鈥听
This is especially important in children who have recurrent strep infections to help听identify the source.听
Thankfully, there are tests your pediatrician can perform in the office to confirm or rule听out strep throat. 鈥淎 quick test that has been available for many years is the rapid听antigen test. However, it can miss up to 30 percent of cases. That鈥檚 why a follow-up听throat culture can be needed, but it can take several days for the results to come back. Fortunately, a test called the rapid strep PCR assay is 95 percent accurate and only takes 20 minutes. This PCR (polymerase chain reaction) test is what our Cornell pediatric clinics use,鈥 says Dr. Sakhno.听
Are there other possible diagnoses?听
Depending on your child鈥檚 vaccine history, your pediatrician may be able to rule out two听rare, potentially life-threatening conditions that also cause a sore throat: e辫颈驳濒辞迟迟颈迟颈蝉鈥攁苍听infection of the small flap that covers the windpipe, caused by Haemophilus influenzae听听type b (Hib); and Diphtheria, another deadly bacterial disease that obstructs breathing听and swallowing.听
Both of these have become quite rare in the U.S., thanks to safe and effective vaccines.听The Hib vaccine, administered to infants as young as 2 months old, protects against听epiglottitis caused by the Hib bacteria. And since the introduction of the DTaP vaccine (the听鈥淒鈥 in DTaP stands for Diphtheria), that deadly disease has all but disappeared in the听U.S.听
A slew of viral infections can cause a sore throat. But, as Dr. Sakhno notes above,听these are often accompanied by a runny nose, congestion and cough, meaning it鈥檚听unlikely that the child has strep when these symptoms are part of the mix.听
How do you treat strep throat?听
Penicillin or amoxicillin taken for 10 days are both highly effective. And, fortunately, if听your child is allergic to these antibiotics, alternative options are available.听
Why is it necessary to take antibiotics for strep throat?听
Beyond the curative powers of antibiotics for strep, it鈥檚 critically important for your child听to take them to prevent both immediate complications and delayed post-strep sequelae (consequences) that can occur weeks after a strep infection. These include the following:听
Acute rheumatic fever听
鈥淎 serious complication of an untreated strep throat is acute rheumatic fever,鈥 she says.听鈥淚t presents as joint pain, chest discomfort, shortness of breath, abnormal movements听and rash, and can progress to rheumatic heart disease.鈥 These symptoms develop in听around 1 percent of untreated strep throat infections and begin a few weeks after the sore听throat. Antibiotics taken for strep throat reduce a child鈥檚 risk of developing rheumatic听fever by 70 to 80 percent.听
Nephritis (kidney damage)听
Damage to the kidneys after a strep throat infection can also occur in some cases, leading to swelling in the arms and legs, high blood pressure and dark urine several weeks post-infection.听
鈥淥nce again, antibiotics taken for the original throat infection will prevent most cases of kidney involvement,鈥 Dr. Sakhno says.听
Infectious complications听
The strep bacteria can also spread to nearby areas of the body as well as the nervous听system and cause:听
- Abscesses in the tonsils听
- Meningitis and other diseases of the nervous system听
- Ear infections听
- Sinus infections听
- Sepsis: a potentially life-threatening condition that occurs when the infection-causing bacteria enters the bloodstream听
Antibiotics help to reduce the risk of a strep throat infection spreading to these听other areas of the body, she says.听
What if my child gets strep throat repeatedly?听
鈥淭here鈥檚 a natural tendency for strep throat infections to decline as children get older,鈥听she says. Still, if your child gets strep often, it鈥s possible that they are a 鈥渃arrier鈥 of听strep, meaning that they continue to have the strep bacteria even when not feeling ill.听This happens in 12 percent of children.听
To help your child鈥檚 pediatrician determine whether your child is a carrier, ask yourself the following questions and discuss them with your child鈥檚 pediatrician:听
- Are these infections truly distinct episodes with a full recovery in-between?听
- Did you administer the complete, 10-day course of antibiotics?听
- Are your child鈥檚 symptoms consistent with a strep infection as outlined听above?听
Ask your pediatrician if your child should be tested when they鈥檙e symptom-free, such as during a well visit. If the strep test is positive, they may be a carrier. The good news is that carriers are very unlikely to transmit the infection to others and are also unlikely to develop strep complications.听
Can home remedies help?听
Supportive measures, such as warm liquids, lozenges, acetaminophen and ibuprofen,听can help your child feel better. But, Dr. Sakhno advises, 鈥渁void viewing these as听alternative treatments capable of fighting the strep infection on their own.鈥听
When is a tonsillectomy recommended?听
Tonsillectomy isn鈥檛 usually indicated, even for recurrent infections, because children听typically outgrow them as they mature. But 鈥渢here may be times when an ENT consult is听recommended,鈥 she says.听
When can my child return to school after a strep throat episode?听
According to the most recent studies, 91 percent of children test negative 12 hours听after starting antibiotics for strep throat.听听Therefore, it is a-okay to听have your child return to school when 迟丑别测鈥檝别 been on antibiotics for at least 12听hours and has been fever-free for at least 24 hours without using fever-reducing听medications such as acetaminophen or ibuprofen.听
