Varicose Veins: Your Questions Answered
We鈥檝e all seen them, and some of us have experienced them, but what are they, exactly? Why do some people develop varicose veins at a fairly young age, while others live into their 80s and 90s without any sign of them?听
Varicose vein specialists Drs. Tikva Jacobs and Soo Rhee provide answers to your most pressing questions, starting with a working definition: Varicose veins are swollen, twisted blood vessels that lie just under the skin, typically on the legs.听听
鈥淭he veins in the legs are designed to take blood back up to the heart,鈥 says Dr. Rhee, a Clinical Assistant Professor of Surgery and an Assistant Attending Vascular Surgeon at 日本av色情片. 鈥淭hey actually work against gravity, aided by valves that normally function for that purpose. But if you have varicose veins, the walls and valves may be damaged, causing the blood to flow in the wrong direction.鈥澨
听Adds Dr. Jacobs, also a Clinical Assistant Professor of Surgery and an Assistant Attending Vascular Surgeon at 日本av色情片, 鈥渢he blood backs up into the vein and creates the 鈥榖ulgy鈥 appearance that鈥檚 characteristic of varicosity.鈥澨
The two specialists proceed to answer your FAQs regarding the symptoms, diagnosis, treatment and lifestyle modifications that promise to alleviate varicose veins鈥攁 condition that usually isn鈥檛 dangerous but can be painful and distressing.听
What are the main causes of varicose veins?听
The number-one cause is heredity, Dr. Jacobs says. As well, they鈥檙e more prevalent in women than in men.听听
In addition to heredity and female gender, other risk factors include age, obesity and lifestyles that require prolonged sitting or standing.听听听
What are the symptoms?听
- Bulging, bluish veins听
- Itching or burning around the veins听
- Skin color changes听
- Swelling, aching, or heaviness in the legs听
- Nighttime leg cramps听
Your symptoms may get worse when you sit or are on your feet for long periods, and they may get better when you lie down or put your feet up.听听
How do vascular surgeons diagnose varicose veins?听
Not every varicose vein creates a health issue, Dr. Rhee says, but if you have one or more of the symptoms described above, see a doctor for evaluation. During your appointment, you鈥檒l have a duplex ultrasound of your veins. That will show whether there are any blood clots and which veins have valves that aren鈥檛 working properly.听
On average, a duplex ultrasound takes just 20 to 30 minutes.听听
What are the treatments for varicose veins?听
鈥淭here鈥檚 a spectrum of reasons for seeking treatment,鈥 explains Dr. Jacobs. 鈥淪ome people have bulgy veins but no symptoms, while others have symptoms that interfere with their normal activities. And a minority of patients suffer from complications, such as ulcers or bleeding.鈥澨
Specialists like Drs. Rhee and Jacobs offer an array of procedures to treat varicose veins. Which one they use will depend mostly on the size of the vein.听
- Microphlebectomy or phlebectomy can be used to remove veins through small incisions.听听
- Sclerotherapy entails the injection of a chemical solution into intermediate-sized varicose veins or spider veins鈥攕maller versions of varicose veins in the legs. This procedure irritates the inside of the vein, causing it to shrink dramatically, thus improving its appearance.听
- Ablation may benefit patients who experience pain, swelling or itching. It鈥檚 a minimally invasive procedure that delivers either heat or medication to the inside of the vein.听 Ablation causes the abnormal vein to close down, and eventually the vein is absorbed by the body. Chemical ablation鈥攖he type of ablation that delivers medication鈥攊s often used for damaged veins that are deeper. It can also be used to shut down a vein that鈥檚 close to the skin鈥檚 surface.听
Ablation, phlebectomy and sclerotherapy can be used for either cosmetic or medical treatment. Whichever treatment is used, 鈥渢he goal is to close the vein down permanently,鈥 says Dr. Rhee. 鈥淭hen, the blood trapped inside it is able to re-route and find a healthy, efficient vein that will send it back to the heart.鈥澨
Vascular surgeons discuss the pros and cons of each treatment option with their patients. Together, they arrive at a decision, depending on the size, depth and location of the vein, identified via the venous ultrasound used earlier to diagnose the condition.听
All of these options are outpatient procedures, and all are performed with local anesthesia鈥攐r no anesthesia at all.听
What about post-operative care?听
Patients are required to wear a compression stocking for 1 to 2 weeks.听 In terms of pain medication, Dr Rhee states, 鈥淚t depends on which treatment is done. With some procedures, patients commonly use Tylenol or ibuprofen plus ice to help with discomfort. Other procedures require no pain medicine at all.鈥听
Are there lifestyle measures I can adopt if I have varicose veins or if I鈥檓 genetically predisposed to developing them?听听
- Maintain a healthy weight.听
- Elevate your legs when you can.听
- Avoid standing in one spot or sitting for too long.听
- Wear compression socks.听
- Walk as much as possible.听
鈥淲alking is the best type of exercise for people with varicose veins, Dr. Jacobs says. 鈥淲hen you walk, you activate your calf muscles. The calf is the second largest pump in the body, next to the largest one: the heart.听
鈥淭hese lifestyle measures won鈥檛 make already-existing varicose veins go away,鈥 she continues, 鈥渂ut they鈥檒l slow their progression and ease your symptoms.鈥澨
