Detecting Prolapse Starts With Symptoms听
The bladder is the most common organ involved in pelvic organ prolapse. Other organs involved may include the rectum, uterus and vaginal walls.听听
When pelvic organ prolapse is present, you may experience the following symptoms:听
- A bulging sensation coming from the vaginal opening听
- Difficulty emptying the bladder听
- Feeling like you may be 鈥sitting on an egg鈥听
- Pressure in the pelvis that worsens throughout the day or when coughing or standing听
- Seeing something bulge outside of the vaginal opening听
By not treating the problem, you can experience discomfort during daily activities.听
鈥淧elvic organ prolapse can have a huge impact on quality of life,鈥 Dr. Rolston says. 鈥淚鈥檝e had patients stop exercising, not want to go out and cease sexual activity because of prolapse.鈥听
Pelvic organ prolapse is diagnosed during a pelvic or vaginal examination. Based on the stage of your prolapse, your provider will review treatment options which include surgical and non-surgical options.听
Conservative Treatment for Pelvic Organ Prolapse听
Your gynecologist or a urogynecologist may provide conservative treatment for pelvic organ prolapse. Regardless of who provides treatment, your options may include:听
- Pessary placement. A pessary is a silicone device that comes in various shapes or sizes to fit inside the vagina. Before using a pessary, you鈥檒l undergo a fitting session. Doing so ensures you can go about your daily life comfortably, walking, urinating and performing other tasks of daily living with pessary in place. Once positioned inside the vagina, the pessary acts like a shelf. The prolapsed organ rests on the pessary, keeping the organs inside the vagina. Every three to six months, the pessary requires cleaning. Depending on the type of pessary you are fitted with you may be able to remove and clean the pessary at home or have your provider do this for you during an office visit.听听
- Physical therapy. Through pelvic rehabilitation care, you can perform special exercises to strengthen the pelvic muscles and help keep the organs in place. This approach is most helpful before the prolapse extends beyond the opening of the vagina and causes symptoms. Kegels are a common exercise for a stronger pelvic floor. To perform these exercises, tighten pelvic floor muscles, as if trying to stop your urine flow. Hold for three seconds, then release. Do 10 repetitions, three times per day.听
鈥淕enerally, we give patients options, and conservative approaches can help many women,鈥 Dr. Rolston says. 鈥淪ometimes, however, we can鈥檛 find the correct pessary size. When that happens, we recommend surgery.鈥听
Surgical Correction of Pelvic Organ Prolapse听
There are two main types of surgery for pelvic organ prolapse reconstructive and obliterative. Both options are minimally invasive and generally are same-day surgery.听听
Obliterative surgery narrows or closes the vagina altogether, preventing organs from protruding. Sexual intercourse is not possible following an obliterative procedure.听
Reconstructive surgery, on the other hand, aims to reposition pelvic organs where they belong. Sexual intercourse is possible following reconstructive procedure.听
Types of reconstruction include:听
- Laparoscopic mesh repair. The surgeon operates through tiny incisions in the patient鈥檚 abdomen. A permanent mesh material is placed on the vagina to hold the pelvic organs in place. This reconstruction can be done with or without the assistance of a robot.听
- Trans-vaginal approach. The surgeon uses stitches to reinforce the fascia. In some cases, the surgeon may remove the uterus (known as a hysterectomy) at the same time.
